Terms and Conditions for the Platinum Dental Surgery Referral Program

1. Who Can Participate

This program is for our existing patients (“Referrers”) who introduce someone new (“Referrals”) to Platinum Dental Surgery.

A “new patient” is someone who has never registered or received treatment at any of our branches.

2. What Referrals Get

Free registration and consultation for their first visit.

A possible discount offered by the clinic manager.

These benefits cannot be exchanged for cash or given to someone else.

3. How to Refer

Fill out the referral form with accurate details.

Incomplete or incorrect forms will not be accepted.

4. Using the Benefits

Free registration and consultation are valid only for the Referral’s first visit.

Benefits must be used within 30 days of the referral being submitted.

5. Confirmation

Both the Referrer and the Referral will receive confirmation via email or SMS after the referral form is submitted.

6. Program Rules

Each Referrer can introduce only one Referral.

7. Changes to the Program

Platinum Dental Surgery can update or end this program at any time without notice.

Changes will not affect benefits that have already been earned.

Anyone found misusing the program may be disqualified.
